ETSU Family Medicine presented HIMSS Davies Award of Excellence

A successful initiative by the Department of Family Medicine at East Tennessee State University to improve collaboration and communication after patients are discharged from the hospital was recognized by the Healthcare Information and Management Systems Society.

ETSU’s Department of Family Medicine, which operates three clinics in Bristol, Johnson City and Kingsport, received the HIMSS Davies Award of Excellence for decreasing hospital admission and re-admission rates and improving transitional care management and care delivery.

When the department recognized that a low percentage of patients were being seen for follow-up appointments after being discharged from the hospital, transitional care clinic interprofessional and interdisciplinary teams were formed to focus on leveraging health information and technology to create a standardized processes for scheduling follow-up appointments for discharged patients.

The teams worked with local hospital systems to implement an admission, discharge and transfer feed into the electronic health record. The new interface allowed the family medicine clinics to identify patients and receive notifications immediately after a patient was admitted into the hospital.

“We are so honored to receive this recognition for the work we have accomplished in patient-centered care through health information technology,” said Julie M. Montgomery, clinical quality data manager in the Department of Family Medicine.

“Our investment in compassionate and efficient care through effective communication skills and standardized processes has led us to excellence in collaboration and innovation. We are making a difference in the quality of care we provide to our patients.”

The HIMSS Davies Award of Excellence recognizes outstanding achievement of organizations that have utilized health information technology to substantially improve patient outcomes and value. ETSU’s Department of Family Medicine will be recognized during the 2019 HIMSS Global Conference & Exhibition in February.

“Receiving this award reminded us that we are on the right track by building up our technological services to support the education we provide to each and every family medicine resident physician and medical student,” added Montgomery.
